TOURNAMENT PROCEEDS ARE DONATED TO CHILDRENS HOSPITAL
8th ANNUAL MEMORIAL TOURNAMENT - MAY 21st-23rd 2010

OSYSA REGISTRATION REQUIREMENTS -you will be required to provide:
1. Copy of State Approved Roster (plus two copies which will be retained)
2. Player Pass Cards (for each player and guest player) Passes MUST BE LAMINATED (Limit 3 guest players- with a maximum of 12 on the roster for U-9, a maximum of 14 on the roster for U-10 and U-11, and a maximum of 18 on the roster for U-12 and above).
3. Coach Pass Cards (for each registered coach) Passes MUST BE LAMINATED
4. Travel Permit (if from outside of Ohio South) -please obtain travel permit online or from your association. We will not have someone available.
5. Guest Player Roster (if applicable) This may be done online if you are from Ohio South using the e-travel system found on www.osysa.com website under forms. Please bring a copy of the approved roster.
6. If you are registering a player who does not have a card, they must bring a picture and copy of a valid birth certificate along with a $20.00 fee.
When and where is registration?
Team Registration for ALL teams will be held at the Kolping Sports Park- 10235 Mill Road, Cincinnati, OH 45251. Registration will be held Friday, May 21, 2010 5:00-9:00 pm.
It is Mandatory that each team send a representative. ALL teams must register Friday night. There will be NO registration at the fields. Players and/or teams which are not compliant with registration requirements will not be able to participate.
Each year our tournament continues to grow. It's not because of the competition that comes out to participate, but because each year teams, families and players understand what our tournament is about. Kolping holds this tournament inorder to remember Seth Stevens as a player and a friend. Therefore, we donate the tournament proceeds to Children's Hospital Hemotology/Oncology Division where Seth was being Treated.
We also, run a Toy Drive so that we can donate Toys to Children's Hospital as well. Each year we are shocked, surprised, and thankful of all the Donations that Teams, Families, Players and Organizations make. It is great to see all different teams come together for a Great Cause.
Last year we had a great success with the donations. Kids from all clubs raised money, selling lemonade or going door to door asking for donations, just to be able to give toys to the hospital. It was a great success.
We do ask that for all teams that are planning on bringing toys- either as a team or individuals to donate to Childrens Hospital, Please click here too see a list of acceptable toys.
